- M-F 8-5
- Complex care, Incoming patients are invite-only
- Seeing patients 16 and up, 8-10 patients per day
- The phone call is shared 1 in 5
- Join 4 other physicians
- EMR is E clinical works
- onsite infusion center
- BLS certification and a strong focus on customer service is required
- Experience in population health management and working with psychosocially and medically complex patients is strongly preferred
- collaborate with a team that includes a nurse practitioner, medical assistant, nurse care manager, clinical social worker, and office coordinator
